One group did twice-weekly supervised heavy strength training — working progressively up to 80–85% of their one-rep max — plus impact work like drop landings, which is a recognized way to stimulate bone. The other group was a waitlist control, continuing their usual routine. Thirty-eight women completed the full nine months.

This wasn’t an unsupervised “go lift heavy” free-for-all. Every participant started with a movement assessment, and the heavier loads were built up to gradually, under a coach’s eye, twice a week for the better part of a year.

What they found

Compared with the waitlist group, the training group came out ahead on every measure tracked:

Total body lean mass: +0.52 kg (training) vs. −0.38 kg (control) — a difference of 0.90 kg (95% CI 0.08 to 1.72)

Leg power output: +70 W (training) vs. −28 W (control) — a difference of 97 W (95% CI 2 to 193)

Lumbar spine bone density: +0.013 g/cm² (training) vs. −0.007 g/cm² (control) — a difference of 0.020 g/cm² (95% CI 0.001 to 0.034)

Strength itself rose 12.6–30.5% across the four main lifts tested (chest press, shoulder press, deadlift, squat)

Every one of those differences was statistically significant, meaning the confidence intervals stayed above zero — this wasn’t a case of a few outliers skewing the average.

What this doesn’t mean

It’s worth being honest about the size and scope of this study. Thirty-eight women at a single clinic is a feasibility trial, not a large definitive outcomes study — its first job was to test whether a program like this was doable and safe in this population, and it succeeded at that. It’s also not a green light to walk into a gym and load a bar to your perceived max on day one. Every participant here worked up to 80–85% of max gradually, with supervision and an initial movement assessment. The “heavy” in this trial describes where participants ended up after months of progressive, coached training — not where they started.

Why this matters for you

The gentler-is-safer instinct during perimenopause comes from a reasonable place — joints can feel different, recovery can feel different, and nobody wants to get hurt. But this trial adds to a growing body of evidence that muscle and bone respond to being challenged, not protected from challenge, and that midlife is not a stage to be trained around but a stage that specifically benefits from resistance and impact work, done progressively.

If you haven’t done any strength training yet, this is a good reason to begin, starting light and building the way this trial’s participants did. If you’ve already been lifting for a while, this is a good reason to stop underestimating what your body can still adapt to, and to progressively challenge yourself with heavier loads — ideally with guidance on that progression, the same way the women in this trial had.

Either way, “heavier” here means gradual and supervised, not a leap to a max-effort lift alone.

If you’d like help figuring out where you’re starting from and how to build a strength routine that actually fits your menstruation cycle and your hormonal stage, book a free 25-minute introductory call and let’s talk through it.

Citation: Whitman et al., STOP-EM feasibility trial, 2026.

As estrogen declines through perimenopause, bone and muscle loss speeds up — and the advice most of us hear in response is to take it easier: lighter weights, gentler classes, less impact.
