It’s just part of getting older. Not much you can do about it.

Here’s what that story gets wrong: a new study just proved you can do something about it — and more than you’d probably guess. You can measurably reverse heart stiffening. But there’s a catch, and it’s the reason this isn’t a “someday” read: the window to act on it is now, not later.

The study (actually, two studies from the same trial)

A research team led by Dr. Benjamin Levine ran a two-year randomized trial with 61 healthy but sedentary adults aged 45–64 — the exact stretch of life when heart stiffening tends to quietly set in. Half were assigned to a structured high-intensity aerobic training program: four to five sessions a week, built mostly around higher-intensity interval training and longer moderate-paced sessions, with just one or two shorter strength sessions layered in, gradually building up over two years under supervision. The other half — the control group — did yoga, balance work, and strength training only, with no dedicated aerobic training at all.

That comparison turns out to matter a lot: the control group’s non-aerobic routine produced no improvement in fitness or heart flexibility, while the training group improved in both. That’s a big part of why it’s fair to credit high-intensity aerobic training specifically here, not “exercise” as a catch-all.

Fifty-three people made it through the full two years. Everyone had detailed heart scans and fitness testing before and after.

Two separate papers came out of this same trial, looking at two different things:

Howden et al. (Circulation, 2018) looked at the heart’s structure — specifically, how stretchy the heart’s main pumping chamber (the left ventricle) was. A stiffer chamber can’t relax and fill with blood between beats as well, which is the root problem behind a type of heart failure called HFpEF — where the heart still pumps normally but doesn’t fill properly. This kind of heart failure is disproportionately common in women after menopause.

Hieda et al. (Journal of Physiology, 2019) looked at something different but related: the body’s moment-to-moment control system — the automatic reflexes that adjust your heart rate and blood pressure in real time as you stand up, move, or get stressed, and how well your heart automatically pumps harder when more blood flows into it.

What they found

The training group’s fitness (measured as VO2max, a standard marker of cardiovascular fitness) improved by 18% over two years. The control group’s didn’t budge.

More importantly, the training group’s heart chamber flexibility improved by more than 25% — a meaningful, measurable reversal of the stiffening that normally comes with age. The control group’s heart stiffness didn’t change.

On the control-system side, the training group’s overall heart-and-blood-vessel coordination improved by 34% — including a stronger automatic reflex for regulating blood pressure and a better ability for the heart to pump harder in response to more incoming blood. The one thing that didn’t change in either group: the stiffness of the blood vessels themselves. So this isn’t a case of exercise vaguely “improving blood vessels” — it’s the heart muscle and its control system specifically getting retrained.

Here’s why aerobic training in particular seems to be the driver: sustained aerobic effort repeatedly sends a bigger volume of blood back into the heart, session after session, over years — which is closer to a stretching stimulus on the heart chamber than a pressure one. Dr. Levine has compared an unused heart to “a rubber band left in a drawer” — it gets stiff simply from disuse, and repeated aerobic loading is what keeps it supple. Strength training works the heart differently (more of a pressure/resistance stimulus), which is likely why the control group’s strength-and-yoga routine alone didn’t produce the same effect.

There was one catch, and it’s the part worth paying closest attention to: the researchers found this kind of reversal was strongest in people who started training before roughly age 65. Past that point, the same program still improved fitness, but the heart-stiffness reversal was smaller. The earlier window appears to be when the heart is still flexible enough to meaningfully remodel itself.

Why this matters in midlife

If you’re in your 40s, 50s, or early 60s, this isn’t a “someday” issue — it’s happening in real time, and it’s also the window where you have the most leverage to change its course.

HFpEF — the type of heart failure linked to this stiffening — hits women far more than men, and menopause is part of why: declining estrogen is thought to accelerate the same stiffening process this study measured. That’s not meant to be alarming. It’s meant to explain why “cardio is good for you” undersells what’s actually going on — this isn’t just about burning calories or feeling less winded on stairs. It’s about the physical flexibility of the heart itself, and there’s a real, closing window to influence it.

What to do about it

This wasn’t a casual “go for a few walks” program. It was structured, progressive, and supervised — four to five sessions a week for two years, mixing real higher-intensity interval work with longer moderate-paced sessions. That’s a meaningful commitment, and it’s worth being honest about that rather than implying a couple of easy strolls will do the same thing.

A few honest takeaways:

If you’re not currently doing any structured aerobic training, this is a strong reason to start now rather than “eventually” — the study suggests the benefit is bigger the earlier you begin.

Higher-intensity intervals were the biggest part of what made this program work, mixed with longer moderate-paced aerobic sessions — strength training was in the mix too, but as a smaller supporting piece, not the main driver.

Building up gradually over months, not jumping straight into hard intervals, is how the study’s participants did it — and it’s the safer way for anyone starting from a sedentary baseline.

The fine print

Source: Howden et al., Circulation, 2018, and Hieda et al., Journal of Physiology, 2019 — two papers from the same two-year randomized trial (Levine lab, Institute for Exercise and Environmental Medicine). Small trial — 61 participants enrolled, 53 completed, ages 45–64, a mix of men and women with no separate results reported by sex — so take this as a strong early signal rather than the final word, especially for how it applies specifically to women. This was a demanding, supervised program, not a casual routine, and results like these take sustained effort over years, not weeks. One more honesty note: Dr. Levine told NPR his team “took these 50-year-old hearts and turned the clock back to 30- or 35-year-old hearts” — a memorable line, but it’s his own characterization in an interview, not the study’s formal statistical conclusion. What the paper itself actually measured is a partial, specific improvement (that 25%+ gain in ventricular flexibility) — not proof that any one person’s heart becomes biologically younger overall.

Where to go from here

High-intensity aerobic training is one lever on heart health — a powerful one, but not the only one pulling on how you feel day to day through this stage of life. Hormones, sleep, stress, and habits all play a part too.

If you want a clearer picture of what’s actually going on for you, my free Menopause Self-Assessment walks through the habits and factors shaping how you’re feeling right now — download it, and if it’s helpful, you can book a free 25-minute call to talk through what it turns up, including where this kind of training fits into your own plan.

You’ve probably heard some version of this before: your heart stiffens as you age, the same way your joints do.
